Looking to build something similar to how Form Publisher works

I have searched all over this form and other resources via Google and I can't find what I'm looking for, so I thought I'd post in hopes that maybe someone would have the answer. I can't do screenshots of what I've done because honestly, I'm all over the place and have started over more times than I can count at this point. If someone is familiar with Form Publisher, then this will make a lot of sense. 🙂

 

Here's what I"m looking for:

 

1. I have a Microsoft Form created that users will fill out

2. I can view the form results in Excel from SharePoint and see each result, line by line

3. I'm looking to figure out how I can take each form submission and dump it into an Excel template so it's easy to read, and ideally I'd like each submission to be created with a unique name.

-There are also formulas in this Excel template.

4. Lastly, if it's possible to email out the newly created file as an attachment, that would be icing on the cake.

 

I've tried using 'When a new response is submitted' from Microsoft Forms, 'When an item is created or modified' from SharePoint, 'When a file is created in a folder' from SharePoint, and even tried some other random tutorials.

 

Thank you for any help or direction.

    Thank you for your response. I have tried this one, but it creates a list in SharePoint and I'm needing an Excel file per each submission. It did not seem like this solution would work for me - is there some way you think I can make it work to create an Excel file per each submission?

    Hi @kristinhope79 ,

     

    As far as I know, we can only insert data into an existing Excel table. You can define fields in the Excel table to store the submission.

     

    Then use Add a row into a table action to insert the submission into the table.

    I finally got it working the way I need it to, for the most part. I couldn't get a PDF created due to some sort of rights issue, but that wasn't worth my time to figure out - I can at least get it to send an Excel document with just the latest form submission attached in my Excel template.  Thanks for the tips.

    Has anyone worked out how to change the date format?

     

    Currently shows at M/d/yyyy - we want DD/MM/YYYY but can't find out how to do this?!
