Salesforce task created after Sharepoint item is created

I'm working on a flow that creates a task in Salesforce after a sharepoint item is created. The issue is that after the item is created and the email is sent out, when I check the opportunity to see if a task has been created, there nothing there. Not sure what I am missing in order to make this flow work. Below is screenshots of my flow:

Could you please share a screenshot of the run history of the flow when an item has been created and the email is sent out?

What is the type of the "NoOpportunity" column in your sharepoint list?

What is the value in the "NoOpportunity" column when an item has been created?

I afraid that the "Create record" action haven't run due to the Condition 2 in your flow.

Do you want to create the record when the value in the "NoOpportunity" column isn't equal to No?

Please share more details so we would try to provide a proper workaround for you.

NoOpportunity is a Yes/No type in the Sharepoint.

Basically, if NoOpportunity is yes, then that means the request doesn't have an OpportunityID attached to item. If NoOpportunity is no, then there is an opportunity ID attached to item. So I want a task to be created afterwards with information from the sharepoint item uploaded to the task in Salesforce.

I was able to figure out the issue and was able to successful embed a task in the Salesforce opportunity. Thank you for all the assistance.
