What goes in the "Item" field in SharePoint Create Item step

I have a simple flow that's triggered from a Forms response and then should Create Item in a SharePoint List.  It's recently started failing and it's due to the Item field in the Create Item step.  What should be in this field (image below)?

    The portion you show in your picture varies from list to list. This area contains fields that have been tagged as required. You will need to look at your list to determine what it is. The asterisk next to the field means it is required. As an example, this is how this action looks in one of my flows:
    As you can see, my values are different as they're dependent on the fields in the list. If you don't know what to fill it in with, change it from required to optional in the list.
