Planner Task updating notes / history log in Dynamics 365

Hi 
I'm currently trying to find a way so that a task in planner can update the notes or history log field in dynamics 365. 

For example if a user is assigned a task in planner and they add notes or complete the task, that information is updated in the Notes / History Log field for an Account in Dynamics 365.
