Adding multiple attachments to a SharePoint List item from Form

I have been after a solution to this for a while with little to no luck. This form has been butchered a few times so it may not make sense at first, but what I am trying to do is take a form submitted with multiple attachments and add said attachments into a SharePoint List for a ticketing system. Please help!

    Confirmed this works if using OneDrive, but the way we have our system catalog our files is through a specific folder on our SharePoint. This is what I have currently trying to adapt your solution to that format.
