I am trying to create a Power Automate that will be using a premium action
Whenever a user creates an entry in a SP list, it will be executing a query and create an entry in an on-prem SQL server
For this flow, should I get a "License By User" ($15 per month) and create the Power Automate or will it require a "License by Flow" ($500 for 5 flows)?
If I get the License by User and created this automated Flow on Item creation in SharePoint, will it work when other users create a new entry in the list?

This is based on the scenarios around how you plan to utilize the license.
If you are actively developing multiple flows every few days or months which use premium connectors, then you can go ahead and get yourself ( or preferably a service account, so that the flows won't stop after you leave the org) a premium per user license.
In case you don't foresee yourself of the team building many flows with premium connectors, then it would be better to go for a "License by flow", as it would save you cost over long run.
At the end of the day, it all depends on how you plan to utilize those licenses after you acquire them.
