ToolIdentifierConflict error for any prompt in copilot studio

My Copilot Studio Agent started behaving strange with "ToolIdentifierConflict" error code. This agent doesn't have any duplicated tools or with tools with same name. 
Please help me figuring out what caused this issue and a resolution to it.

    It would help a lot to see the Agent. Its possible you are triggering 2 tools or 2 instances of the same tool so its conflicting.
     
    The error itself isn't enough, as I can see it gives you multiple errors, which means we should be able to see in the Activity as well as the Flows being used, what its doing.
     
    This really appears to be it triggering multiple Tools which is possible, hence why we need to see how you have, what you have, configured etc.
     
    Is this part of a Generative Node or simple question node?
