Power Automate and MS Form - Create form result into PDF (with Picture)

Hello There! 

I am trying to build a flow for Microsoft Form. 

Every time a form is filed in, it then gets sent to certain email address based on criteria. This email will contain the PDF file of the result and this PDF would also sometimes include photos. 

I am attaching a screen shot of this flow, and the error I am receiving in the PDF files. Please can some one help!
