Start and Wait for Approval looks Different

I recently created a flow based off of a trigger when a MS Forms is submitted. I have a series of three "Start and Wait for Approval" each one with a condition of Outcome = Approve to move to the next approver. Problem is, I have just noticed that the approval UI has changed. Sometimes it will prompt an approver to open up a new window in Power Automate, other times, the approval can be done directly in the email.

 

I also had someone approve (with the in-email approval) and it said they request was accepted, but when I look at the flow running path, it still says it is waiting. The timing is also incorrect (it's still in the afternoon and yet the timestamp says it was approved in the future evening?)

Any ideas as to why the same approval requests look different at random times? Also, why it seems to be stuck on this condition when it was clearly approved?

    Hi,

     

    I am afraid the times may be displayed under UTC. About why the Approval action is not completed yet, have you configured the action as "Everyone must approve"? Perhaps there is more than one approver.
