Flow for Forms response to Sharepoint list is not working (no errors)

Hello!

I've set up a simple Power Automate flow that when a form is populated, it should create an item on a sharepoint list.

There are no errors and I'm able to save. The status is "on" but when I submit tests through the form, the list is not updated. I've tried over two days just in case timing was an issue but still there's nothing under the flow's run history. Any suggestions on what I can look into in order to see what's broken?

Thanks!

You might be using a very old template.

You don't need the loop.  See my example below:

The form isn't being picked up if it's not being ran. Can you double check that the form you're submitting is the correct form you've got linked to the flow?

If it isn't available from the dropdown, click custom --> paste the form ID from the forms url into the box.

    Hi Scott,

When I rebuild like yours, I don't have Response ID as an option:

And when I select the only option I have, it reorganizes the screen automatically:

    Thanks,
    Nicole

@Babybots  The form is available in the drop down and yes, it's the correct form, thanks.

@Babybots  Actually, it looks like you've suggested the right thing - apparently we have two forms named the same and one is owned by me and one by another person. Can I not select a form owned by someone else (but shared with me) for my flow? Maybe that's an internal security thing?

    Thanks so much!

I've been tricked by this as well, it's the only reason I figured it may be the cause.

I think it will show up under your forms only after you've done the custom method of adding the id of the form that was shared with you.

Thanks - where exactly do I get the ID of the form?

Open the link of the form you're wanting to connect  --> grab everything after the "ID" in the link (EX: https://forms.office.com/Pages/ResponsePage.aspx?id=XXXXXXXXXXXXXXXXX) --> XXXXXXXXXXXXXXX is the ID for this form)
