Adding attachment to existing sharepoint List

Hello,

 

I am trying to add an additional attachment using the "add an attachemnt" action to a SharePoint list that already exists and contains attachments. When I Run the flow, it creates 5 or 6 flows that fail with the error "The specified name is already in use. The document or folder name was not changed. To change the name to a different value, close this dialog and edit the properties of the document or folder", But one of the 6 flows does succeed, and it works. What is causing the repeated failed attempts with this error message, and how can i resolve the issue.

    @Anonymous unless we can see a screenshot of your flow there's not much advice we can give. Also, you said "When I run the flow, it creates 5 or 6 flows" - a flow doesn't create other flows so you're going to need to give us more information.

    Sorry for being vague. I am creating a pdf file in one drive, and then adding that file to the sharepoint list. I am then including that attachment in an email I send. By the 5 or 6 flow message, what I mean is that by the 5 or 6 flows comment is that once the flow is triggered, 5 or 6 duplicate flows run with the same purpose as the initial flow, all with the  same error, when they should not exist at all
