Can we build the Model Driven app with connecting On-Premise Sql database server ?

Model-driven apps (as per this post and this reference guide) are DataVerse only, however you can certainly have a Canvas app via on-premises SQL.

One way to connect to an on-premise SQL database from a model driven app is to develop a virtual entity. This is an object that appears like a Dataverse table but connects to an underlying SQL table. This requires some C# expertise and some knowledge on how to secure your database.

The 'low code' method as @WarrenBelz suggests, is to build a canvas app which you would then embed into your model driven app.
