I need to create an approval flow when an item is created/ modified

can advise what is not right here?

Scenario

1) User submits data into SP as a new item, it will trigger to approver 1 with attachment in the email body once approved, it will route to 2nd person to input (approved) with attachment in the email body and approver 1 status info; email to user

2) User submits data into SP as a new item and triggers to approver 1 approver 1 with attachment in the email body , rejected by Approver 1, The user needs to reinput missing items, and the approval route restart
