Flow duplicating lines in SharePoint List

I have created an approval workflow which writes back the outcome of the approvals to a SharePoint list. However instead of getting 1 line entry per document I am getting 2, anyone had this before? Can't see anything in my Flow that would enable duplicate line entry to prevent it. 

 

The approval flow does allow for more than the one approver - the first line seems to only show details of 1 approver and the second has them both concatenated. 

 

Any help/suggestions greatly appreciated. I am losing the will to live with it now...

    Hi @Anonymous ,

     

    If Approval type selects “Everyone must approve”, then Responses will be an array, and each element contains Approver’s related content.

     

    "Responses Approver Response" and "Response Comments" similar Dynamic content contains the approval details of single Approver contained in the item currently traversed.

     

    If this is the case, then depending on your Flow configuration, multiple items will be created, which will record the each approver information and approval details for the approval.
