Link Form to prebuilt AI model for invoice processing

I have been working on Prebuild AI models of Invoice processing and recently encountered with an issue that I came across.


On the left, I have the prebuilt AI invoice processing model and on the right I have the normal form that I added up. I tried to link the values from extracted information to that form, it went through. The problem that is coming over is, While running the App, I am not able to see the form on the right. I am not sure what to add to the "Item" property of the forms in order to get it displayed over to run the app. This is what I see when I run the App. 



Can anyone suggest something, please?

    Can I see how do you extract these fields in your app please?

    Hi, thanks for stating up and clearing this. I think, you are able to see the form on the right side when you play the app is because the form is set to NEW mode. When I set the form to EDIT mode, I cannot see it.
     
    Does form has to be in EDIT mode or NEW mode in this as I need to store the details captured by Invoice processor to the sharepoint list.
