So is there a way to ensure the flow runs as soon as a file is added to the folder?
Seems no way. Actually, I got similar problem as yours. Even though I just shared flow with another person. Then I started flow through that account, the new flow history delayed for few minutes which is unnormal. Then, I ran flow one more time, the result delayed showing up, however, the previous result came out.
While researching, I realized this might be a by-design bug. The shared flow will cause slow performance. But there is no more detailed information clarified for this scenario from official documentations.

To avoid such kind of issue, it will be better if we can set up same flow for specific users manually instead of using shared flow. I understand it will cause redundancy. But at current stage, there is no any other good idea. I also recommend you go to UserVoice website to post this issue.

What happens if 2 different users are logged into the sharepoint site at the same time - how does the flow decide to run based on the user?
The scenario you described is related to concurrency control of flow. There is no limitation for the number of flow runs that can run at the same time by default. So, the flow will run for each at the same time in normal.
