Sometimes when I rename an icon it goes from the default loo to a 16x16 (ish) black version, it only sometimes happens but when it starts happening it will keep doing it very repeatably

Would it be available for you to share a screenshot of the problematic icon?
Better with the Session ID when this issue occurred.
Session ID could be found under:
File->Account->Session Details

The one on the left is the default icon as it is placed, the one on the right is after changing the name from icon5 to icon_5, not that both the icons were excatly the same when placed the only difference is one has been renamed,
