Form Name into Sharepoint List

So, I have a form and am using PowerAutomate to populate a sharepoint list upon submission.  However, I want to include the name of the form in the submission, example below (rather than use the sessiontitle field)...how can I do that, as I can't see it as a dynamic element?

    If you're not planning to change the name of your form, is there a reason you cannot manually type in the name of the form? If you do happen to change it, you would need to update your workflow. Otherwise, I don't see that this information is returned in a Forms response.
