Map Forms ID/Name/Title to Sharepoint List

Hello,

I have mapped my forms information to a sharepoint list except for one: the Name/ID/Title of a form. Is there a way to add it/map it to the sharepoint list?

    Hi @Hardesh15 ,

    So I need the Form Name to be mapped to the Sharepoint list. It is because I need to grab it when I need to make a certificate of completion of quiz. My certificate gets information from a sharepoint list where all the answers to the forms quiz will be saved.

    Ex. my first form quiz is named History Quiz - that name should also be reflected in the sharepoint list (if possible)

    @CatPagulayan  So you have one SharePoint list and many Quiz forms which are mapped to SharePoint List. if this is the case- You can use one compose action and pass static value like- History Quiz and then use compose output for SharePoint list. Similarly you can do for other Quiz forms.

    oh okay I see.. I thought there is like a field in Forms such as "Title" that can be quickly mapped out 🙂 Seems there is no way but to create that compose action. Will take that as the solution. Thank you!
