I need to create a application form like this and I would like to ask if I need to use Powerapps or forms for this? IF I need to use Powerapps can someone please tell me how to add that second section with checkmark Lägg till Vårdnadshavare 2 and the button Kopiera adress från which means copy address from the first vårdnadshavare.
Its better you use Powerapps. You may use Checkbox and dropdown/Comboboxes

Thank you for your reply. Will I be able to post that form externally so that people can access it and submit a request. I am talking about anonymous access. Where the submissions will be saved? Today, when someone make a request, that request is saved locally on the server.
If external anonymous are involved, the go for forms instead of powerapps.

However with anonymous , go for forms as you would need email for the powerapps external users
Here is the exact scenario. That form is accessed with a link on our website. When people use that app form and when they send response all informations is saved in a file. I need solution so that we can post a link on our website so that people can use the form to send their application or request. This form should be accessed by any user regardless who he is or where he is residing. What to use to accomplish that?

As suggested earlier, you this Microsoft forms for this scenario, you can send them the link from forms so they can fill.
