Automating an email response after a multiple choice question is answered.

Hello,

I am currently building a mandatory COVID-19 sign up form for employees using Microsoft Forms. There is a question in the form that asks if they have experienced any symptoms (which are then listed as multiple choice options).

The way I want this flow to work is to send an email to the safety supervisor whenever someone that submits the form and has selected that they have experienced any of the symptoms stated before. This email would have the title of "COVID-19" alert or something along those lines and the content of the email itself would be the name of the person that submitted the form ,the date it was submitted, and the symptom/s that they have selected (information that is asked in the form).

    Use MS flow and follow steps-

    1. MS flow action- When form response is submitted - select form id.

    2. Use Get response details.

    3. Use condition action and check all option as- "Are you experience......"[value] is equal to "your option-1" (remove double quotes )

    4. in similar fashion you can add multiple conditions and use operator-OR 

    5. Once condition is true send email.
