Createdby Date/Time in 24hr format needs to be in 12hr format

Hi All, 

I created a flow that runs hourly to create a list of tasks that are still pending and the time they were created and then post them into a channel. I have everything working except the "Time" is showing in the 24hr format and I want it in the 12hr format. 

 

Any help would be appreciated.

    I thought that worked but now it is showing a different time. I tested it at 11:58 and it is showing as 04:58 now.  Any ideas?
