Where to begin creating a user-driven questionnaire?

Hi there!!

I would LOVE some guidance on if/how to create a user-driven questionnaire with the following requirements. When I say user driven, I mean that users will have the ability to enter their own questions with associated blank fields for entering answers from clients.

Questionnaire that has branching if/then questions
Questionnaire can have added questions on-the-fly during Discovery
Each question has:

Checkbox for designating questions as either N/A, Parking lot, or Requirement

N/A - question is struck out
Parking lot - DevOps item creation/Excel document list creation

If box is checked, a new field is available to enter the DevOps item title and description, and a button to create the item.

Requirement - DevOps item creation/Excel document list creation

If box is checked, a new field is available to enter the DevOps item title and description, and a button to create the item.

The entire questionnaire is auto submitted to an excel file for data collection to eventually produce industry-focused standardized questionnaire templates.

Have you tried using Microsoft Forms? I reckon all your requirements can be done with them. Could be a good start rather than building a PowerApp. https://www.office.com/launch/forms.

Thanks for the reply. I did look into Forms, but I didn't see how it could allow a push to create DevOps items based on fields from the form.

If this function is possible, I would appreciate any recommended articles/videos on how to set it up.

You could use Power automate to retrieve the result from Forms and then create an Item in Azure Devops.
