Attaching Activity to Contact

Hi all

I’ve build a flow when a response is submitted on form a record is created in an activity entity and is working fine. Now after creating a record in custom activity it needs to be attached with the existing contact record with same email matching to it. I’m unable to do it, is there any way some one help me on this
