SharePoint List not creating new item when form is submitted

Hello, 
We have an issue when a form we created via Power Apps is not creating a new item or entry after we submit. What it is doing instead, it only updates the first line item repeatedly after each submission. This is a brand new form, with a brand new list. 

Additionally, we have a power automate flow behind it that is triggered by a new item is created in the List. However, the flow hasn't been triggered yet because no new is technically created in the List. 

 

Troubleshooting: After I cleared that first entry/item and try to resubmit a new form again, the List won't create any items anymore.

    Hello,

    I got this Problem to! SharePoint isnt creating a new Item when a new Form is submitted. is there any issue with Sharepoint? 

    My Flow is working fine.

    Hi @MurphyCooper ,

     

    It looks like you're updating the same existing single item rather than inserting a record once the form is submitted, and once the same record is deleted, it's throwing an exception.

     

    Make sure : Form Mode FormMode.New

    Could you please tell me what is the default mode of your form?

    If your form's default mode is Edit, please change it to New.
