Sudden error in D365 CRM linked flow

Hi everyone,
I have been running a flow for more than a year now that is triggered when a row (in the lead table) is updated. The flow then adds a new row to the phone call table which is linked to the lead in the trigger - using the lead id. Now this step is causing an error and I can't work out why.

Action 'Add_a_new_row_-_Phone_call_activity' failed: Creating Entity with an invalid parent. Entity: PhoneCall, ReferencingAttribute:regardingobjectid

Is anyone else seeing anything similar? The flow was working fine on Friday, something seems to have changed over the weekend. I have this same sequence running in multiple flows and all are failing.

I notice for some reason there prefix / is missing from phonecalls as the lookup identifier

Thanks for the suggestion @Michael E. Gernaey - unfortunately it still gives the same error.

As per your statement, lead is been associated with your newly created phone call activity. If so, then regarding object should contain lead entityname not phonecall.

    Thanks.

Thank you @vejai! That worked. It's strange how the flows have been running like this for over a year with no issue but I will update them all now.
