I have an approval that has more than 5 custom responses. Approval, Exception, Denied, Concern for A (for simplicity sake only using letters), Concern for B, Concern for C and Concern for D. Because the approval responses are limited to 5, is it possible to branch/cascade a response within the approval email?

I don't believe it's possible, the approval email has just a response and comments field. Maybe you could utilize the comment field somehow - ask the user to input a specific value and then process it, but that would probably affect the user-friendliness of the flow.
