Assistance Required for Custom Copilot Flow & Connectivity Issue

I hope this message finds you well. I am reaching out to seek assistance with an issue I am experiencing while creating a custom Copilot. I have developed an issue assistant bot and connected it with an action flow in Power Automate. However, the flow is not being triggered by Copilot as expected.

The intended function of my custom Copilot is to create a new item or ticket in a SharePoint list and send a mail notification to the assigned user when a user enters a prompt related to an issue ticket. The output should indicate that a new issue has been added to the list. Unfortunately, the Copilot is not functioning as desired. Whenever I enter a prompt, it immediately responds that the issue has been added, but upon manual verification, the item is not created. Occasionally, the item is created but requires manual connection.

I am seeking a solution to overcome this issue. If anyone has experience with this or can provide any insights or solutions, I would greatly appreciate your assistance.
