I'm using a GPT prompt to read a pdf document (I have also tried this with text), and classify the document into 3 types. The flow runs well the first time or a few times. But it breaks down and I keep getting this error. I have tried a lot of fixes but nothing seems to work. I have even tried a Recognize text in image or document connector that works perfectly, to deliver text to the prompt. This is so disappointing, I am looking to build an invoice processing solution that would rely heavily on prompts.

    This is a new one for me.
     
    Can you please try what it says first... delete it and recreate it? Its worth a shot.
     
    Next, can you please open the run in the old UI (just click the new UX off top right toggle).
    Then open the Action in this run here.
    Let's get a copy of the Raw Inputs and Raw outputs
     
    if you don't mind sharing those please. Paste them in, please don't use pictures so I can copy them to review them.

    have the same issue started from 07/08 as GPT5 was introduced. It is happening with o3 gpt and when i am running a manual test with old design in power automate prompt is working. which is very strange. After this flow is not working anyways only during manual test

    Thanks for replying. This is the 3rd time I'm replying, they keep disappearing :/
     
    I deleted and added the connector back several times. It only works once or twice then goes back to the error. 

    Also my experience is exactly like @CU10070821-0's. Worked with the old designer tests, but failed at the very first actual run. 

    I've attached the Raw inputs and outputs.

    One more thing. No matter what I do, this parameters keeps coming back.

    Same issue began today for me, with GPT-4.1 selected as the Model. Have deleted and added back the AI Builder in an attempt to solve, but did not work. Has anyone found a solution yet?

    We have the exact same problem. I believe this is an error in the editor generating wrong code, which result in a weird item property "item/source" after the workflow is saved/loaded.
     
    If you export a flow to a zip file, you can see the wrong code which partially includes escaped JSON from the item/source property. Sometimes the item/source property is duplicated. I don't have an example at hand, perhaps I can post it later.
     
    After readding the AI builder prompt it sometimes works once or twice until the error reappears. We use the new version of the flow builder.

    Since 07/08/2025 when gpt5 was introduced ai prompt got this bug. Please this issue is gives a big problem with the same error. Be sure each day more people would come in with it. Please take a look at it. Delete the action doesnt help and nothing else, try it yourself multiple times and you will see. Same issue everyone has + it gives this weird item source field which has never been there. This should be resolved by Microsoft

    i have found a workaround but still it has to be fixed by Microsoft.
    Once you have everything ready for you script and for your prompt save it all. Then delete the prompt action and add it back again and we need to not touch prompt itself you can add text input but dont get inside of it or you will get the same error. After you added text input field you can paste your outcome of the prompt and never touch it again. I have tested if you are not touching anything after first save especially prompt action it is working for multiple times not just once
