Microsoft Forms creating duplicate items in Sharepoint List

I have created a flow so when there is a new response in Forms, it creates a new item in Sharepoint list.  However, two items are created one with the correct information and one with information (email address) missing.  This is causing issues with some of my other flows linked to the Sharepoint list.  Have uploaded a screenshot of the flow.

 

Does anyone have a solution?

    @matthewlea I can see you've created it from the Forms template. I know that because it adds that illogical and erroneous apply to each. In the last 5 years I've been working with Power Automate Microsoft have never corrected it and it causes problems like the one you've experienced. To save work copy your create item action to your clipboard then create a new instant flow from blank and immediately after the when a new response is submitted trigger add the get response details action. Then add the Create item action from the clipboard.

    Hi @RobElliott I tried your solution but the issue is still persisting.   I tested it twice and on the second occasion it created three items in the Sharepoint list 🙃

     

    See screenshot below of the output in the sharepoint list, and underneath for the new flow.

    I have this same issue. I have duplicates in a sharepoint list which is being populated from a form using power automate. I have no loops in my power automate at all, it basically takes the items from the form and adds to the list..

     

    No idea how to debug because it should just work.

    I'm very new to Power Automate, but i have a flow like this that connects forms to sharepoint list and i do not have this problem. It looks like this:

    It looks like yours pretty much, might it be another flow you have that causes it?

    Ive created many of these forms before, and usually working fine... very weird.

    ive created a completely new form and it seems to have solved the problem.. what a waste of time
