Receiving two approval requests from power automate

Hello,
 
I'm receiving two approval requests from power automate when a request is submitted using the following flow. The basis of the flow is to have the approval email routed to the Manager of the employee, which is inputted by the submitter when the intake form is completed. I cannot figure out why there are two approval emails being generated.
 
I placed a screenshot of the SharePoint list and the latest run of the flow at the very bottom.

    I don't see anything in your post that would indicate the flow would send you two approvals. Can you show the expanded settings of the approval action, specifically these settings:
    Are you specifying an account for the requestor? If not, and the flow runs on your account and you are sending yourself an approval, you are probably seeing the notification you get as the requestor of the approval, as well as the actual approval. Can you share a screen shot in the approvals center that shows the two approvals you said you are getting? Can you actually approve both?
     
    In the approvals center, the approvals you have sent will be under the sent tab, while pending approvals will be under received:

    Below is a screenshot of the approval center. Only one approval is shown for each run which makes sense. But outlook is receiving two approval notifications. In the additional screenshots below, you'll see two approval emails received and a screenshot of each. They appear as identical images, but they are the two separate emails you see in the inbox. I added the screenshot of the expanded section you mentioned at the very bottom. 
     
    The flow is running under my account - which if this is the cause, I'm ok with it, I just don't want it happening when someone in the business submits the request for themselves.
