Power Automate: Flow Triggers and Teams Integration

I have set up a Microsoft Form and wanted to use Power Automate so that when that form is filled in, it triggers a response to create a 'ticket' with all the information provided and will show on a board created in teams using the Planner app. 
 
Whilst I seem to have set these triggers up, Teams is showing a notification but none of the information provided from the Form. 
 
I'm a novice to this so was hoping someone could provide some insight so I can get this set up for my studio request management.

    It sounds like your Power Automate flow is triggering correctly, but the Planner task isn’t pulling in the form responses. The key is to map each Form response to the Planner fields:
    In Power Automate, after the “When a new response is submitted” trigger, use “Get response details” to pull all the answers.
    In the “Create a task” action for Planner, make sure you insert the dynamic content from the Form responses into the Title, Description, or other fields.
    Test the flow with a sample submission and check that the fields appear correctly in Planner.
    Right now, Teams sees a notification because the task is created, but if you don’t map the fields, it shows empty.

    I recreated your scenario in Power Automate to better understand the issue. The flow structure itself is correct, but the behavior you are seeing is expected when form responses are not mapped correctly.

    Below is the correct setup and explanation.

    Step 1: Trigger – When a new response is submitted


    • This trigger only detects that a form was submitted

    • It provides only the Response ID, not the form answers

    •  

    ✅ This step is configured correctly

     

    Step 2: Action – Get response details

    ✅ This step is mandatory to retrieve the actual submitted answers

     

    Step 3: Action – Create a task (Planner)


    • The Planner task will be created even if no form data is mapped

    • You must explicitly insert dynamic content from “Get response details”

    • Use form questions for:

      • Task Title

      • Task Description

      • Any other task fields

    ❌ If dynamic content is not selected from Get response details, the task will appear blank

     

    Why this happens


    • “When a new response is submitted” does not return form answers

    • “Get response details” retrieves them

    • Planner does not fail if fields are empty, so the task is created without form data

    This is a very common scenario when first working with Forms, Power Automate, and Planner.

    Recommendation

    Populate the Title or Description of the Planner task using outputs from Get response details so the form information is visible in Teams and Planner.
