Data extracted from PowerApps using flow does not appear in the correct fields on my word form.

Hi,

 

I created an canvas app to capture daily data which then populate a word form with flow. Everything works perfectly but the issue is that none of the data appear in the correct fields on my word form. The field ID's in PowerApps and Word are exactly the same. What am I doing wrong?
