Forms: Compatble data sources?

What's the key to getting Input Forms to work with Custom Connectors? I have a custom connector that supports GET, PUT, and POST calls, however Forms don't work with any of the methods... 

 

Is there documentation that specifies how the Connector has to work in order to be compatible with Forms?
