Power Automate Flow creates duplicate entries in MS List

I have created following flow to autofill the MS List with response details collected from MS forms.

The flow is triggered once the ms form is submitted, but it creates 2 entries for a single response.

    Your flow looks good as is - but - just in case - can you post a screenshot of the flow in edit mode, showing the config of each? I don't see any reason why it would duplicate.

    Why is your Form ID showing as text?  You should be able to select it from a dropdown. 

    I don't think this is the issue, but, it is strange.

    Are you sure the submissions to the list are identical?

    It was not showing in the dropdown options, thus I entered as a custom value.
    Yes the entries are exactly same.

    I.... have no idea. That's a bug.  You're getting one form, one response, and creating one item. Unless the user is sending the exact same content twice through the Microsoft Form, it shouldn't be creating a duplicate.

    I'm wondering if it's something to do with the new "allow users to edit form response"  capability. Do you have that flipped on for users in your form?
