How to post and automate fillable form to be emailed

Hello,

 

I created a fillable word doc form with a submit button to be sent via email.

 

I wanted to see if there's a way to post form (macro enabled template) somewhere so the staff in my organization can easily access to fill out the form and have it sent to me.

 

Or if there's an automation that I could set up.

    Thank you! This is definitely helpful. Although, I don't have the premium version to set the convert to word or pdf so when the form is printed, it only has the basic layout of the questionnaire. It's a health form so we sort need it to be printed as an actual form layout. Not sure how this could done.

    Hi @mizzmeeshelly,

     

    Just like @anupam8555 said, using MS Form will be a better choice to fill a form that your staff in your organization can easily access to fill out.

    Then you could create a Flow that with trigger When a form is submitted. If one staff submit his form, get the Response details and append them into the Word template to create a new word file and send to you as an attachment:

    Hello @mizzmeeshelly ,

     

    You can use Microsoft Forms or Forms Pro for your requirements. You have build similar forms in Microsoft forms which you can send it out to your employees for fill up. Once they fill out it can come back to you. 

     

    You can set approval process with power automate also.
