Approvals generating without trigger in middle of the night - Bug?

I'm seeing an unusual behavior and looking for help in identifying if this is a bug or if it is something else. I have been making automated cloud flows for my company for about 2.5 years and this behavior was first noticed on May 03, 2024 when a flow (flow #1) that had been in use for months "suddenly" sent out 5000 new approvals for the same task. I have around 30 flows that actively operate and about 10 with approval loops that were created in the past 2.5 years, none have ever shown this kind of behavior before.

This means that the execution of the create and wait for approval step within the flow ran in infinite loop without the flow starting each execution. There is no record in the flow's history that it executed on the day that the new approval records were created in the dataverse Approvals table. Each approval had a new key but the same content of one previously executed flow process that was still "running" (waiting for an approval response).

I had discounted the instance as a fluke and moved on with life after deleting all the approvals that were generated that were not tied to running flows.

This past weekend this happened 3 more times. On June 8th one different flow (Flow #2) from the one on May 3 (Flow #1) executed this behavior twice with 2 different currently running flow sequences (10000 new approvals, 5000 of approval for task A, 5000 of approval for task B). On June 9th the same flow as on May 3 (Flow #1) executed this behavior again for a different currently running flow sequence (5000 new approvals for task C).

Both of these flows have been created since the "new designer" and AI was integrated early this year. Both were built using the with the new designer deactivated.

Has anyone else experienced a situation like this or have any advice of what to look at to find the cause of the problem since there are not a similar 5000 new flow "runs" that are happening so it would indicate it is not the flow but rather is something else going on?
