Microsoft Forms Data Extraction

I created a Microsoft Form and am trying to set up the following flow: Trigger flow when a new response is submitted -> Get response details (entire response) -> Add a row into a table. 
To set this up, I submitted a response and then downloaded a copy of the excel sheet and moved it to a separate location on Sharepoint to ensure that the table perfectly aligned. I then named the table so I could refer to it in the last action. When I submit a response, the table expands, but no data appears. What do I have to do to get this seemingly easy flow to work?

    It sounds like your 'add to row' needs to be nested inside an 'apply to each' condition.

    Have you told the flow what to input in each row that's added?
     
    Would be good to see what your actions look like.
