Get URL to Lookup Column Item from SharePoint list

I'm building a flow where I pull SharePoint list items using Get Items. 

 

I want to be able to create an email table where one column will be a hyperlink to take the user to the lookup item.

 

How can I get the URL of the Lookup column's SharePoint list entry?
