I'm receiving an email that Start an Approval Action Failed. Please check the values assigned for each action.
The flow runs successfully, other than receiving this message. Any help would be appreciated!

Can you show what error you are getting?
What value are you assigning to the Assigned to the field? Is it claims, name or email?

Hi Sundeep. I receive the following email message after the flow runs successfully:
This one is an easy fix.
I think you only want it to email if there is an error.
In that case, Click ... on your GetMyProfile action and choose "Configure run after"
Set it to run only if the approval action fails
