Could you please share a screenshot of your flow's configuration?
Do you use the "Start an approval" action to send the approval email within your flow?
I suppose that you want to make a response to approval emails from your email body (Actionable Message), is it true? I have made a test on my side and don't have the issue that you mentioned. When I click Reject button within the email body, the Comments section shows up well.
Currently, the Actionable Messages is available only on the Outlook web app and on the desktop version of Outlook 2016. For the latter, you can install the latest update to get Actionable Messages functionality. Please check if the version of your Outlook client meets the requirements for actionable messages.

In addition, if you want to change the text of the approval buttons (Approve and Reject) within a approval email sent via the "Start an approval" action, I afraid that there is no way to achieve your needs in Microsoft Flow currently. If you would like this feature to be added in Microsoft Flow, please submit an idea to Flow Ideas Forum:

If you want to change the text of the approval buttons, I think the "Send email with options" action of Office 365 Outlook connector could achieve your needs. I have made a test on my side and the flow's configuration as below:
The flow works successfully as below:
But if you use the "Send email with options" action to send the approval email, you could not add comments to this request.
