ReportApproval flow for team member to complete a form, this goes the manager to complete further details, then to HR for final approval.
I want to create a workflow where a team member completes an MS form to request to work from another location with details of the request (including location / dates / reason / work health and safety checklist).
The form responses are then sent to the team member's manager, who completes further details and a checklist to ensure the team member's request is compliant.
All responses are then sent to HR for final sign off.
Is this possible? I have an approval workflow up until team leader, but unsure how to add further responses to the original form. The checklist that the Team Leader needs to complete is significant (about 40 questions). Do I need 2 separate forms and one flow?
