I’d like the form to flow into an email using the format laid out below:

Email (Fields in Yellow should be static):

From: [capture field from SUBMITER NAME]

CC: [capture field from EMAIL ADDRESSES OF INDVIDUALS YOU WOULD LIKE CC'D ON OUR RESPONSE]

    To:LICHP.0000.NA.US.EN.MSF.RMD.CS.T02.SPT.00.EM@css.one.microsoft.com

Subject: [New Form] + [capture TITLE field from intake form]

Attachments: [capture ATTACHMENTS RELATED TO INQUIRY]

Body: The output of the (HTML?) table created in the link you provided

    Hi shariq,

About the error “Bad Request”, from the error message it seems that you didn’t enter valid values to the list. What was the value you entered?

What are the column types in your list? Please share more details about the SharePoint list. I will try to reprodcue this issue on my side.

Please check this documentation about troubleshooting a flow for a reference. Flows fail if a setting in an action of the flow doesn't function as expected. In this case, the error message contains Bad request or Not found, or an error code of 400 or 404 appears:
    https://docs.microsoft.com/en-us/flow/fix-flow-failures

    Best regards,
    Mabel Mao

@shariq - did you get a chance to apply @v-yamao-msft's suggestions to adapt your Flow?

If yes and you found that solution satisfactory, please go ahead and mark this thread as "Solved". 🙂

    Happy Flowing,

    -AT
