Simple tickable/eSignature 1 page pdf creation and email as attachment

Can anyone help?  basic scenario - a student requests an emergency loan and, upon relevant evidence, we pay it.  I have a flow where we (staff) complete an MS Form which does all the requisite adding to SharePoint and Excel and checks whether it's been paid and sends emails, however, we need the student to agree to the terms and conditions.  If we could prepopulate MS forms than that would be ideal, however, as you cannot do that, we do not want yet another form (especially as relies on human input) but would like a simple pdf that the student receives with their details (from the aforementioned MS Form) already added so all they have to do is tick the box that says 'yes please', provides their signature (which could also be a tick box rather than an actual signature) which they return via email and we save in an outlook folder for auditing purposes.

Have plenty of flows that create a word using uploaded template and can easily convert to PDF to send as an attachment but cannot make the tick boxes 'tickable' ... can I use DocuSign instead?  If so, how??

I've tried to work our how to use the connectors but they just don't make any sense to me.

I have the same question (0)

    Have you taken a look at achieving this via Approvals at all? Upon the Response being submitted to the Form, you could trigger an 'Approval' to the student (using the dynamic content of the Form Response if needed) asking them to Approve/Reject the T&C's?

    Thanks for that suggestion - issue is that it is the student not a staff member who needs to agree to our terms and conditions so they are not 'approving' it just confirming they agree to it

    Apologies - replying to 2 at once as well as fighting of the remains of Freshers Flu ... such fun!

    I have tried to work out how to use Adobe DocuSign but just can't figure out how to do it from submitting a form with the students details to adding the details into the pdf to then send to the student to sign ... unless I've missed something?

    @COLLETTLJ - If the 'Approve/Reject' terminology isn't applicable you can add your own custom response options (detailed here) - so you could change these to 'I Agree' and 'I Disagree'?

     - to the approval request ... can you use dynamic content to send an approval request?  I didn't think you could?

    @COLLETTLJ  - Yep, your bespoke text would go in the 'Details' field on your 'Start and Wait for an Approval' action and you can also include dynamic content. This field can also be formatted as per the guidance here.

    @AlexEncodian @BenUnlox

    Thank you for suggestions - I do already use approvals within my current flows so was aware of possible 'personalisation' but only internally with staff between departments and hadn't even considered possibility of using it for students, but it appears to work using dynamic for assigning to student emails (organisational ones only) with 'I agree' or 'I disagree' as options.  I also used markdown to format the body of the approval and, once approved, save it as a pdf to add to file for auditing purposes.

    It's made the whole process so much quicker!
