Creating a app to upload attachments

I want to create a simple app that will allows users to upload documents which then can be stored as attachments on a sharepoint list. 

 

I have tried to create an app with a connection to a SharePoint list but if I insert the attachments control there is no option to upload files. Am I missing a step?

 

I dont want to use a form purely becuase I would like to desgin the canvas.
