Using first function for attachment

When using the first function when attachments are being used, I am getting errors but not sure why and it's becoming very frustrating, any help would be appreciated.

    So i've been told the below, but still don't understand, feeling pretty useless 

     

    The attachments should have more than one attribute, you use one for the file content and the other for the file name. You also want to have the attribute outside the first brackets so that this way it is identifying a ingle value rather than the whole column
