logging a response from an approval flow to a sharepoint list

I have an approval flow.  Once the user selects an approval (refect, approve, Need more info), I would like to log the response in a sharepoint list that has been created with responses from a form.

 

how do I reference the response from the approval flow then log it in the correct column in my sharepoint ?

 

Thanks for any suggestions and especially examples.

    thank you for taking the time.  it seems straight forward.  however, after my " Create Approval ", i do not get the dynamic value, "Response". in your example.

    Hi @FAli2,

    Do you want to store the approval history in a separate list?

     

    For example, the request ID (renamed Title column), approver name, approver response, comments, and response date/time.

    Each approval response should be a separate item in this list. Add the ‘Create item’ action after every approval action and create new item with the outcome.
