How to create a Sequential approval flow with option to Approve , Reject and Need more information.

For example 3 levels of approval
Each Level user will get email notification with 3 options - Approve, Reject , Need more information
- If the users at each second level Approve, it goes to the next level or
- else if user asks for Need for more information it goes back to the previous level
- or if the user rejects the approval then the process stops
This should continue till all three approved it.
