Gallery selected radio button data only need to my collection

Hi, Every one.
I have one gallery with Radio button. I need only selected radio button gallery item to store my collection.
without empty radio button gallery data.

See above image only i have selected gallery first item radio button (update/cancel). Not 2nd item.
now i need to create collection for only gallery item 1 with all information.

What coding i need to put in button

I am confuse with this more than 4hrs. support me

Now only i am find the solution of my issue after long time.
