Error 403 during approve from Outlook

I have many Power Automate flows that have been running normally for two years and include approvals, without any issues. However, starting two weeks ago, users suddenly began receiving a 403 error when using Outlook. It's worth noting that the same users can still approve through the Power Automate app. Is there any solution to this strange issue?

    I am not 100% what your environment looks like but, we had an issue like this.
     
    Easiest problem with 400 errors on outlook approvals is that it timed out and the response point is no longer active.
     
    Secondary issue and the one we ran into is that we are on a secure network and the user it was sent to is not within the active tenant to allow them to reach the graph api and reach the response point for the email notification.
     
    Third option for a problem could be the fact that the flow that is housing the response point is having an error/running going past the response point and even have lateral branches with emails with options on it were causing breaks.
     
    As for a solution start using adaptive cards. Then if it breaks its usually extremely obvious in the JSON response and you don't have to worry about it.
